Black Dog BJJ – Waiver & Membership Agreement

By joining Black Dog BJJ, attending any class, open mat, seminar, or event, you acknowledge and agree to the following terms.

1. Assumption of Risk

I understand that Brazilian Jiu-Jitsu (BJJ) and other martial arts activities involve physical contact, intense physical exertion, and the risk of injury. This includes, but is not limited to, sprains, strains, bruises, joint injuries, chokes, muscle tears, concussions, and in rare cases, more serious injury or death.
I voluntarily choose to participate and fully accept all such risks, whether foreseeable or not.

2. Release of Liability

I hereby release and hold harmless Black Dog BJJ, its owners, coaches, instructors, members, agents, and representatives from any and all liability for personal injury, illness, death, property damage, or loss arising from participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Medical Disclosure

I confirm that:

• I am in good physical condition and have no known medical conditions that would prevent safe participation.

• I will inform the instructor of any relevant medical conditions or injuries before training.

• I will seek medical clearance if advised to do so by a healthcare professional.

If I sustain an injury or feel unwell during training, I will stop immediately and inform an instructor.

4. Code of Conduct

I agree to:

• Follow all instructions from coaches and staff.

• Show respect to training partners at all times.

• Tap (submit) when in a potentially dangerous position and to release partners immediately when they tap.

• Never intentionally harm another participant.

• Report any injuries or medical issues to the instructor immediately.

• Represent Black Dog BJJ positively both inside and outside the gym.

5. Hygiene & Safety

I will:

• Wear a clean uniform and maintain personal hygiene.

• Keep nails short and remove all jewellery before training.

• Not train if I have any contagious illness, skin infection, or open wound.

• Follow all safety protocols as instructed.

6. Photography & Video

I understand that training sessions, seminars, and events may be recorded or photographed for marketing, social media, and educational purposes. By participating, I consent to the use of my image unless I notify the gym in writing.

7. Acceptance of Terms

I confirm that I have read, understood, and agree to this waiver and membership agreement.
I understand that by joining Black Dog BJJ and participating in any activity, I am legally bound by these terms.

Black Dog BJJ – General Membership Agreement

This Membership Agreement sets out the terms and conditions of membership at Black Dog BJJ. By joining and participating in any activity at the gym, you agree to the following:

1. Membership Types & Payments

• Memberships are billed monthly in advance via the payment method provided at sign-up.

• Payment is due regardless of attendance.

• Prices are subject to change with at least 30 days’ notice.

2. Cancellation Policy

• You may cancel your membership at any time by providing a minimum of 30 days’ written notice (email is acceptable).

• The 30-day period begins from the date we receive your written notice.

• Any payments due during the 30-day notice period will still be collected.

• Failure to provide the required notice will result in an additional month’s membership fee being charged.

3. Membership Holds / Pauses

• Membership freezes may be granted in exceptional circumstances (e.g., injury, medical issues, extended travel).

• Requests for a hold must be made in writing and approved by management.

• Membership holds are granted at our discretion.

4. Code of Conduct

Members agree to:

• Follow all instructions from coaches and staff.

• Treat all members with respect and train in a safe manner.

• Maintain good personal hygiene and wear clean training gear.

• Refrain from behaviour that may endanger others or disrupt classes.

Violation of these rules may result in suspension or termination of membership without refund.

5. Liability Waiver

Your participation is subject to the Black Dog BJJ Waiver & Release of Liability, which forms part of this agreement.
By attending any class, seminar, or event, you confirm you have read, understood, and agreed to the waiver.

6. Communication

We will communicate important updates (e.g., class schedule changes, closures, events) via email, social media, or in person at the gym.
It is your responsibility to ensure we have your up-to-date contact information.

7. Agreement to Terms

By joining Black Dog BJJ, you confirm that you have read, understood, and agree to abide by this Membership Agreement and the Waiver & Release of Liability.
